Wellness Frontiers Award

hlc wellness awardHLC created the Wellness Frontiers Award to promote best practices and draw attention to existing wellness initiatives that demonstrate excellence and quality.  Eligible applicants are non‐HLC member organizations of any size from the governmental, public, or private sectors (or a partnership that combines multiple sectors) that are implementing an evidenced-based wellness program that prevents disease and improves the well‐being of a population.  Community groups with multi‐group support are especially encouraged to apply.

The program or intervention must demonstrate:

  • Cost‐effectiveness
  • Evidence‐based foundation
  • Measurable results (outcomes metrics are preferred, but process metrics will also be considered)
  • Replicability

2019 HLC Wellness Frontiers Award Recipients:

Community Health Alliance – Healthy Living Program was recognized on April 16, 2019.  The CHA’s mission is to bridge quality care gaps in the community, and its Healthy Living Program focuses on helping patients achieve and maintain a healthy weight.  The program has a multi-faceted approach that includes promoting healthy habits, involving a coordinated team of providers, and establishing community partnerships.  It has shown effectiveness in helping children avoid a lifetime of costly health problems by building healthier lifestyles and reducing obesity.  There is a growing list of partnerships across the community that provide bonus classes such as guided hikes and cooking demonstrations and upon graduation participations receive a bike from the Sparks Kiwanis Club Bike Program.

Waianae Coast Comprehensive Healthcare Center was recognized on December 2, 2018.  WCCHC is a holistic community health center that serves more than 37,000 patients.  In order to improve access for the community, there has been continued integration of primary care, behavioral health, nutrition, dental care, diagnostic services and pharmacy within the same clinic.  The Health Center has worked to move away from crisis-driven care in order to lower costs and improve quality of care.  It has also maintained cultural traditions and aligned its values with those of the patients to help them achieve complete wellness.

California Health Collaborative was recognized on November 17, 2017, for its wide variety of health and wellness programs across the state of California.  The Collaborative has established strong partnerships at the local, regional, and statewide levels, enabling its programs and services to be linked into broader systems of prevention and care.  It is part of over 50 programs that target mental health, nutrition, diabetes, smoking cessation, early detection of breast and cervical cancer and much more.  The Collaborative is committed to addressing the health needs of Californians, and places special emphasis on addressing the needs of the underserved, especially those who have limited access to resources affecting their well-being, and face barriers related to culture, language, income, education, gender, geography or immigration status.

Well-Ahead Louisiana was recognized August 22, 2017. Well-Ahead Louisiana’s mission is to promotes chronic disease prevention efforts in Louisiana by providing a coordinated investment of prevention funds; establishing basic, community-based health interventions; and strategically targeting systems-level health interventions to the residents of Louisiana in a more streamlined and efficient way.  The Wellspot program is an initiative that acknowledges worksites, hospitals, schools, child care centers, colleges/universities and restaurants that have demonstrated that they are committed to improving wellness outcomes.  Places and organizations that meet certain wellness benchmarks can be designated as a Level 1, 2 or 3 WellSpot.

Avera eCAREAvera e-Care Services was recognized for its work to bring a variety of telehealth services to the people of South Dakota. Avera’s system provides around-the-clock access to a broad range of healthcare services – from emergency care to pharmaceutical services to intensive care to long-term care. The initiative has garnered hundreds of millions in savings, reached nearly a million patients in eight states, with expansion on the horizon, improved quality by utilizing best practices with all of the health systems with which it collaborates, and improved access for millions of patients with chronic conditions and other diseases.

B’More for Healthy Babies

Healthy BabiesB’More for Healthy Babies (BHB) was recognized for its work to incorporate prevention and wellness into the lives of pregnant women in Baltimore, particularly low income women in minority populations.  Co-led by the Baltimore City Health Department and Family League of Baltimore City, BHB partners with more than 100 organizations to implement a range of programs, including exercise classes for women with young children as well as nurses making home visits encouraging parental health literacy.  With each additional year of implementation, BHB sees more significant results.  In 2012, the infant mortality rate dropped to its lowest point ever in Baltimore City: 9.7 deaths per 1,000 live births.  This is the third year in a row that the infant mortality rate has dropped.  Up until 2009 – the year BHB was launched – the city had one of the highest infant mortality rates in the U.S.  Additionally, health disparities between African American and White infants decreased by 40 percent from 2009 to 2012.

The Institute focuses on programs based on three pillars: health and wellness, education and sustainability.  The award was presented as part of a panel discussion on how Lake Nona fits into the broader healthcare model at the STREAM Health conference.

WeViva

wevivaWeViva is a non-profit organization whose mission is to provide accessible and affordable fitness and nutrition programs to people in low-income communities.  WeViva targets adults in the communities and provide the classes on-site and free of charge.  A team of bilingual, certified, group fitness instructors and nutritionists go where they’re needed the most to provide mobile fitness and nutritional programs, and all the health benefits that go with them, regardless of income and socio-economic status.  Classes are conducted by professional fitness instructors and nutritionists and have anywhere from 10-60 participants in each class. Classes are held onsite at apartment complex or communities, so participants do not have to travel to our classes.

North Carolina Farm Bureau Federation is a private, non-profit, grassroots organization that has been actively promoting farm and rural issues since 1936.
Healthy Living for a Lifetime™ is the Farm Bureau’s innovative approach to addressing immediate health care needs of rural North Carolinians while fostering awareness of healthy lifestyle choices that will result in long-term health improvements among vulnerable populations.  The initiative provides rural North Carolinians with free health screenings, educational materials and a path towards a healthier lifestyle. This initiative succeeds through close collaboration with local rural health care providers, including hospitals and clinics, in a manner that will strengthen their capacity to serve their communities.

okc_million_logo_w_peopleRecognized for successful efforts to elevate the fitness of its citizenry.  After Men’s Fitness magazine ranked Oklahoma City as the eighth fattest city in America, Mayor Mick Cornett challenged his residents to lose a total of one million pounds. Nearly 50,000 visited a website set up for citizens to track their efforts and by January 2012, the city met its goal. Cornett also built on interest in the weight-loss effort to get support for environmental changes that make the healthy choice the easy choice. In 2012, Oklahoma City was ranked as the country’s twenty-third fittest city by Men’s Fitness.

Community Care of North Carolina

CCNCRecognized for its commitment to wellness in serving the state’s Medicaid population, particularly the high quality of care it delivers to patients in rural areas. Community Care of North Carolina (CCNC) is a public-private partnership uniting regional networks of physicians, nurses, pharmacists, hospitals, health departments, social service agencies and other community organizations. These professionals work together to provide cooperative, coordinated care through a medical home model.  CCNC’s Informatics Center supplies care alerts and real-time patient data, and its community-based, population management approach has yielded significant health improvements and cost savings.
